The general inquiry form collects first name, last name, email address, phone number, preferred contact method, and reason for contacting us. Do not submit diagnoses, medical records, insurance member IDs, or other sensitive health information.

How we use inquiry information

We use submitted contact details to respond to your inquiry. We do not sell this information. We do not load advertising or analytics trackers by default.

This is not a patient portal

The public website is not a HIPAA-compliant patient portal. Prescriptions, insurance cards, and medical records should only be exchanged through separately approved secure channels.
